Insurance client registration form, required (for Medicaid/Badgercare clients also!)
If you are paying through private or public insurance (Medicaid, Badgercare), please complete this form prior to your first appointment, if possible. For the sake of saving time and paper, we recommend that you complete and submit the insurance client registration form online. Simply go directly to larsenbilling.com, then click on “Parents” and then on “Verification of Benefits.” You will see instructions for completing the form. After you submit this form and verification of benefits is completed, that information will be sent both to you and your midwife.
Larsen Billing is a wonderful company that advocates for homebirth families and midwives all over the U.S. They charge $15 for verifying your benefits, which is a necessary process and requires this form. You have the option of paying the $15 fee online, directly to Larsen Billing, or paying your midwife.
